I understand that participation in the program involves physical activity and the inherent risks that come with such activity. I certify that my child is in good health and is able to participate in the program.
I understand that the soccer academy and its employees are not responsible for any injury or loss that may occur as a result of participation in the program, unless it is due to their negligence or intentional misconduct.
I agree to hold harmless and indemnify the soccer academy and its employees from any liability, damages, or costs that may arise as a result of my child's participation in the program.
I understand that the soccer academy may take photos or videos of my child for promotional or educational purposes. I give my consent for my child's image to be used in this manner.
I agree to pay the registration fee for the soccer academy program and to comply with any rules and regulations set forth by the academy.
